  • If you have more than one scheme or additional voluntary contribution (AVC) arrangement with ReAssure, you need to complete this form for each scheme or AVC arrangement.

In scope scheme authority to connect

Please read the following information and complete the Authority to connect declaration below.

By providing your scheme information in this form, you are authorising ReAssure to connect and provide data to pensions dashboards on behalf of your scheme. To do this your scheme’s data will be uploaded to Equisoft Limited and stored in Pension Fusion (UK-based AWS Cloud) and from there the data will be sent to the pensions dashboards system.

By authorising ReAssure to share data with pensions dashboards and Equisoft Limited, you as Data Controller are agreeing to the terms specified in this form, namely for ReAssure to:

  • Supply scheme and member data to Equisoft Limited as noted above.
  • Connect and upload relevant scheme and member data to pensions dashboards on your behalf.

Please note in respect of the service provided by ReAssure as set out in this form:

  • ReAssure will not amend or verify the data before it is submitted to pensions dashboards.
  • ReAssure will not be responsible for ensuring the data is correct and complete.
  • We reserve the right to charge you a reasonable fee in relation to (1) resolving any query raised by The Pensions Regulator or (2) any data correction exercise in relation to the submission of the data to pensions dashboards. You can contact us to discuss how we can help on such work and the associated charges separately.
  • Any request for connection on a non-standard basis will be dealt with individually and may be subject to charge.

On behalf of each of the Trustees of the Scheme, I confirm that the Trustees authorise and agree to ReAssure connecting to pensions dashboards on their behalf to provide scheme and member data on the basis set out in this form. In addition, the Trustees confirm the details supplied in this form are correct.
